Enrollment is an easy 3 steps process and we will be with you every step of the way.

  • STEP 1

    Initial Phone Call

    • Start the process by calling us at 718-397-0000. One of our friendly and knowledgeable counselors will help you get started with the process.

    • Our advisor will help to obtain authorization from the patient’s Medicaid insurance.

  • STEP 2

    Completion of Required Paperwork

    • We’ll send you several medical forms and explain what needs to be done.

    • These forms would need to be completed by your doctor.

  • STEP 3

    Completion of In-Home Visit

    • A member of our welcoming team will setup an orientation appointment at your convenience.

    • We will meet in your home with both the patient and the caregiver.

    • We will make sure that you and your caregiver understand how the process work.

Completing Your Setup

We will set the caregiver schedule, and introduce the care team coordinator.

We will make sure that caregiver is added to the payroll to get paid.
